Fit For Duty Rx, LLC operates as a Management Services Organization (MSO) and health and wellness marketing company. We are not a medical practice, healthcare provider, pharmacy, or insurance company. Our business activities relevant to this website include:

• Telehealth referral services: We market and facilitate access to physician-supervised care and wellness programs, including primary care, urgent care, behavioral health, dermatology, medical weight loss, hormone therapy, testosterone replacement, sexual health, and hair and skin programs, through our clinical partners, including Leg Up Recovery and its affiliated physician group Beluga Health, P.A., and National Telehealth Providers (collectively, the "Clinical Partners"). All clinical decisions, prescriptions, membership enrollment, payment, and medical care are provided exclusively by the Clinical Partners and their independently licensed healthcare providers. Fit For Duty Rx, LLC does not prescribe, dispense, provide medical services, or process payment of any kind.
• Corporate wellness programs: We offer employers and organizations access to wellness benefit programs including telehealth access for their employees. These programs are offered as voluntary employee access programs and do not constitute employer-sponsored health benefits under ERISA or applicable state law.
• Supplement store and DNA testing: Our supplement store operates separately at fitfordutyrx.health, and DNA testing is offered through our partner at fitfordutyrx.ordermydna.com. Each is governed by that site's own Terms and Privacy Policy.

2. Acceptance of Terms

A physician-patient relationship is established only between you and the licensed provider through the Clinical Partner's platform, not with Fit For Duty Rx, LLC. All membership enrollment and payment occur on the Clinical Partner's platform after you click through from this website.

5. Care, Wellness & Telehealth Services

Independent Medical Practice

All care and prescription services referenced on this website are provided exclusively by independently licensed healthcare providers through the Clinical Partners' telehealth platforms. Fit For Duty Rx, LLC is not a medical practice, does not employ or direct physicians, does not prescribe medications, and does not provide medical advice.

Compounded Medications

Compounded medications are prepared by state-licensed 503A compounding pharmacies and are not FDA-approved finished products. The FDA does not verify the safety, efficacy, or quality of compounded preparations. Prescriptions are issued only after a licensed provider independently determines clinical appropriateness for the individual patient. Payment does not guarantee a prescription.

FDA-Approved Brand-Name Medications

Where clinically appropriate, the Clinical Partners may also facilitate access to FDA-approved brand-name medications dispensed by standard licensed U.S. pharmacies. Brand and trademark references on this website are property of their respective owners. Fit For Duty Rx, LLC is not affiliated with, sponsored by, or endorsed by the manufacturers of any FDA-approved brand-name medications referenced on this website.

Clinical Partner Compliance

Our Clinical Partners maintain HIPAA compliance and LegitScript certification. These certifications belong to our Clinical Partners and their associated entities, not to Fit For Duty Rx, LLC. We do not have access to your protected health information.

Results & Risks

Individual results vary. Not all patients will qualify for treatment. Treatment decisions are made solely by the prescribing physician. You should discuss all risks, benefits, and alternatives with your provider before starting any treatment program.

Fit For Duty Rx does not share employee telehealth participation status with employers. We make no guarantee regarding insurance premiums, claims outcomes, or cost savings.
